Semi Independent Living Services (SILS) – This program provides support to individuals with an intellectual disability who live in their own home or apartment. Specific services provided are based on individual need and are funded through each county’s SILS budget.
Hourly services – waiver funded – In-home services are available for individuals who are eligible for services through the DD, BI and CADI waivers. These services are provided for children, adolescents and adults who live in their family home or their own home or apartment. The amount of support provided varies greatly and is based on the individual’s needs.
Residential support – waiver funded – These services provide up to 24-hour a day support for individuals through the DD, BI, CADI and EW waiver programs. Services are provided in homes that are licensed as Corporate Foster Care (CFC) sites by the MN Department of Human Services and in unlicensed homes that are owned or leased by the person receiving services. These services are typically shared living arrangements. The amount of service provided is based on each individual’s needs. We provide this type of support for both children and adults who have a variety of service needs.
Residential support – ICF/IID – These services provide 24-hour a day supports to individuals who qualify for ICF/ID funded services. The Home For Creative Living in Windom, MN provides services for both children and adults. Windom RAI and Luverne RAI provide services for adults with intellectual disabilities. All of our ICF homes are physically accessible.

When home care is no longer an option for an older adult, the next choice is often assisted living. Assisted Living services offer the safety and security of 24-hour support and access to care. Habilitative Services Inc provides two options for individuals seeking elderly care. Located in Lakefield, Minnesota, Doman Rose Place and Rosewood Villa cater to individual needs by providing assistance in daily living activities that can no longer be met by personal abilities and/or supports that are not available at home.
Located in a residential area of Lakefield, Minnesota, Doman Rose Place is a 24-hour awake-staffed Assisted Living Plus community for 55+.
Doman Rose Place has 21 single or double occupancy rooms, that provides a comfortable and homey place to call your own. The facility has a group dining area and common areas throughout the facility for social and recreational activities. We are located close to downtown, local churches and the Lakefield Senior Center.
Doman Rose Place is an assisted living, residential option for seniors who want or need help with some of the activities of daily living. Doman Rose Place will work with the tenant and his or her family to develop a personalized plan of care that meets the individual’s needs. Our well trained staff are competent, friendly and caring. Located in a Lakefield, Minnesota, Rosewood Villa is an ideal solution for seniors who value their independence, but wish to have help or oversight with some of their daily activities.
Rosewood Villa is a four unit Senior Housing. Each one bedroom apartment contains a kitchenette, living space, separate bedroom and a bathroom with an accessible shower. The central communal space of Rosewood Villa contains a laundry room with supplies, full kitchen, dining area, and a great room with a fireplace and television that is available for your family gatherings, card club or church group. We want you to continue with the groups you have always enjoyed.
At Rosewood Villa, tenants have as much independence as they want with the knowledge that personal care and support services are available if they need them. There is a dedicated, competent team of staff, including the assisted living aid, the registered nurse, LPN’s and housing manager that will assist each tenant on developing their individualized plan of services.

Guardianship or conservatorship can be provided on an hourly basis and may be paid individually or by a county agency.

Tools for Change – Six video-based trainings for people with disabilities that focus on building skills and knowledge in self-determination, self-esteem, self-worth and dignity.
Are You Hurt? I Can Help! – This first aid skill building training will help individuals recognize basic terminology and techniques used in first aid, such as how to call 911, identifying a hurt individual and learning how they can help.
Intro to Self-Advocacy – Through this training, individuals will explore the beliefs, principles and meaning of self-advocacy.
This Is Your Right- Using a combination of interactive discussion, participation and video, individuals will explore the connection between knowing their rights, understanding responsibilities that follow rights and identifying necessary supports.
